Documents Required
Please submit the claim form together with the following documents within 14 days of the occurrence:
- Medical Report (at your own expense)
- Medical Certificate(s)
- Original Medical Invoice / Receipts
- Certified True Copy of Death Certificate and / or Post Mortem Report if the occurrence resulted in fatalities
