Documents Required
For medical claim relating to your domestic helper, please submit the claim form together with the following documents within 30 days of the occurrence to CTPIS at claimsdept@sg.cntaiping.com
- Finalised Original Hospital Inpatient Medical Invoice / Receipt
- Medical Report
- A copy of Work Permit
- A copy of Passport (if applicable)
- A copy of Death Certificate (if applicable)
- A copy of the Inpatient Discharge Summary given to patient upon discharge
- A copy of Employment Contract
