Procedures
- You are hospitalised for more than 6 hours and/or have a day surgery performed
- You have paid for the medical bill(s) incurred
- Please submit the completed and signed claim form, including the supporting claim documents to your agent, broker or directly to CTPIS at claimsdept@sg.cntaiping.com within 30 days from the date of event. Please ensure that you have indicated your preferred mode of payment in the claim form as well
- CTPIS will assess the claim
Documents Required
Please submit the claim form together with the following documents within 30 days of the occurrence:
- Finalised Original Hospital Inpatient Medical Invoice / Receipt
- Medical Report
- A copy of NRIC / Employment Pass / Special Pass / Work Pass
- A copy of Passport (for Insured Person chooses to seek treatment aboard)
- A copy of Death Certificate (if applicable)
- A copy of the Inpatient Discharge Summary given to patient upon discharge
